Community Nurse Specialists (CNS)
Our CNS team are specially trained to provide care to people living with life limiting conditions, along with support for their carer, in their own home or place of residence.
This means they can offer advice on pain relief, symptoms, your condition and medication. They are also there to offer emotional support to help you and your family cope as you progress through your illness and treatment.
CNS will liaise with you and the Hospice team to introduce you to our medical staff, physiotherapy, Day Hospice, Occupational Therapy and the Patient and Family Support Teams, along with information on home care services and finances.
All of this is to help maximise your quality of life and assess your needs to provide the best quality of care possible at the right time and the right place for you and those close to you.
